The need for long-term care often arises as a result of an ongoing condition or disability, an acute event such as a stroke, or a cognitive impairment such as Alzheimer’s disease. Many of these cases present challenges in functioning and daily living that may be difficult to accommodate within the home setting. That is where our team steps in. We provide around the clock care to meet both the medical and non-medical needs of our patients. At the core of our practice, we believe in optimizing safety, independence, and quality of life, and we do this by coordinating services on an individualized basis for each of our patients.
Our long-term care services include:
- 24-hour supervision
- Assistance with Activities of Daily Living (ADLs) such as dressing, feeding and bathing
- Assistance with Instrumental Activities of Daily Living (IADLs) such as housekeeping, meal preparation and medication management
